Care navigation enables improved access to primary care.

If patients are seen by the appropriate clinician or service this will reduce pressure on GPs, allowing them to see the patients that they need to see the most.

Effective navigation is a key element of delivering coordinated, person-centred care and support. Right care, right place, right time.

Nominate Your Chosen Pharmacy

Whilst visiting your regular pharmacy, sign up for electronic prescribing or ask one of our receptionists to register your chosen / nominated pharmacy.

Changing Your Chosen / Nominated Pharmacy

You can change your nominated pharmacy at any point in several ways, simply by:

  • Visiting the alternative pharmacy and signing up for electronic prescribing with them.
  • Asking one of our receptionists to amend your chosen pharmacy, or
  • through the online portal under the medication section and then click on ‘change pharmacy’.

Please note: You cannot make your initial nomination online.
